BNHC understands that people returning to the community from incarceration have unique health needs and may not receive wraparound care that addresses their medical, behavioral health, substance use disorder, and health-related social needs. The health center’s Brockton Reentry Care Collaborative is designed to support patients reentering the community and ensure their health and social support goals are met.
Health Care Behind the Wall
BNHC offers the following medical inreach services to individuals within 90 days of their expected release date at Plymouth County Correctional Facility:
- Establishing primary care with BNHC for those returning to Brockton and surrounding areas
- Linkage to primary care outside of BNHC for people returning to other areas in the Commonwealth
- Scheduling follow up care post-release
- Case Management with BNHC’s Reentry Coordinator
- Educational groups

Health Care After Release
BNHC offers community-based care for anyone returning to the Brockton-area from a period of incarceration in its Reentry Clinic, which offers the following services:
- Primary Care
- Behavioral Health, including counseling and psychiatry
- Substance use disorder treatment & harm reduction services
- Infectious Disease testing and treatment, including HIV, Hepatitis C, and tuberculosis
- Support with barriers to care and a healthier lifestyle, such as transportation, food, and more
